OculiRX is a screening-grade, provider-reviewed visual-acuity test. This form asks for a conversation about using it in your practice. It does not enroll a practice, sign a pilot, or make the test available for clinical use.

Share the practice name, your role, the state where you practice, and a work email. We store that request on the same list as other provider interest and follow up from the OculiRX team.
